I, understand and acknowledge the risks associated with participating in the activities organized by Warrior Revival. In consideration of being allowed to participate in these activities, I agree to the terms and conditions outlined below:
1. Assumption of Risk: I understand that the activities organized by Warrior Revival may involve inherent risks, including but not limited to physical exertion, strenuous activities, potential for injury, accidents, and exposure to unpredictable elements and circumstances. I acknowledge that these risks may result in personal injury, illness, property damage, or even death.
2. Release of Liability: I voluntarily and willingly assume all risks associated with participating in the activities organized by Warrior Revival. I hereby release, discharge, and hold harmless Warrior Revival, its members, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may occur as a result of my participation in these activities.
3. Medical Condition: I confirm that I am physically and mentally fit to participate in the activities organized by Warrior Revival. I understand that it is my responsibility to consult with a healthcare professional if I have any concerns about my ability to participate safely.
4. Code of Conduct: I agree to abide by the rules, guidelines, and instructions provided by Warrior Revival during the activities. I will act responsibly and respectfully towards other participants, staff members, and property. I understand that failure to comply with the code of conduct may result in my immediate removal from the activities.
5. Consent for Emergency Treatment: In the event of an emergency, where immediate medical treatment is required, I authorize Warrior Revival and its representatives to seek and obtain medical assistance on my behalf. I understand that I will be responsible for any associated medical expenses.
6. Governing Law: This agreement shall be governed by and interpreted in accordance with the laws of the relevant jurisdiction. Any disputes arising from or related to this agreement shall be exclusively subject to the jurisdiction of the courts in the same jurisdiction.
I, hereby grant permission to Warrior Revival to record, videotape and photograph my image and/or voice to be used during Warrior Revival Events and Activities.
I understand that the photographs and/or video recordings may be used for promotional, educational, or other purposes deemed appropriate by Warrior Revival.
I also understand that the photographs and/or video recordings may be used in various publications, including but not limited to, newsletters, websites, social media, advertisements, and press releases.
I acknowledge that Warrior Revival owns all rights to the photographs and/or video recordings and that I will not receive any compensation or royalty for their use.
I hereby waive any right to inspect or approve the final product in which the photographs and/or video recordings appear, and I release and discharge Warrior Revival and its representatives from any and all claims and demands arising out of or in connection with the use of the photographs and/or video recordings, including without limitation, any claims for defamation, invasion of privacy or infringement of moral rights.
